Employment Application – Ban the Box

The Employment Application is a form that applicants complete to indicate their personal information, employment history, education information, and other job-specific information when they wish to be considered for employment. This form also contains a section with the employer's conditions of employment for the applicant to review and sign. Important note: this form DOES NOT require an applicant to disclose any felony conviction(s), which is compliant for employers operating in "Ban The Box" states.
